EverThrive Illinois is proud to announce our leadership of the Care Coordination Working Group for the Illinois Department of Public Health’s Maternal Health Multi-Sector Strategy, an important opportunity to turn bold ideas into lasting systems change for families across Illinois. Building on our 2025 report examining what would be needed to implement universal care coordination for pregnant and postpartum people, this effort advances a key statewide recommendation: creating a universal risk assessment, referral, and care coordination system that begins in pregnancy and continues through the postpartum period.
As the working group lead, EverThrive IL is working with IDPH to convene health care, public health, government, and community organizations as partners to shape a shared vision, a leadership structure, and a practical roadmap for implementation. Together, we will ensure pregnant and postpartum people, especially those most impacted by inequities, can seamlessly connect to the clinical, emotional, social, and economic supports they need to thrive.
EverThrive IL is also centering community voice throughout the broader Maternal Health Multi-Sector Strategy. By partnering with pregnant and parenting people, caregivers, and families from communities across the state, we are helping ensure recommendations from all the working groups are informed by lived experience and grounded in what families truly need. Community members will help shape priorities, strengthen solutions, and guide next steps for statewide action. This work has the power to transform how Illinois supports families during pregnancy and beyond, strengthening continuity of care, closing long-standing gaps, and building a more just and affirming health care ecosystem.
